Job Summary
Jubilee Health Insurance Limited is seeking a proactive and experienced Senior Wellness Coordinator, Population Health to design, implement, and evaluate impactful population health programs. This role will focus on improving the health outcomes of Jubilee's diverse target populations through strategic interventions and data-driven insights. The Senior Wellness Coordinator will play a key role in developing strategies to manage communicable and non-communicable diseases, analyze population health trends, and drive initiatives for cost reduction and improved care delivery.
Duties & Responsibilities
- Design, monitor, and report on the implementation of scheme-wise disease-specific interventions for both communicable and non-communicable diseases, ensuring effective tracking and evaluation.
- Develop and maintain comprehensive population health profiles for all insurance schemes, recommending appropriate control measures based on data analysis.
- Advise on innovative population health initiatives and care delivery models aimed at reducing healthcare costs and minimizing variations in service provision.
- Develop clear guidelines, practical tools, and relevant indicators for effective disease surveillance at the insurance scheme level.
- Analyse, systematize, and disseminate wellness program-specific data to support the development of comprehensive Wellness Departmental reports for informed decision-making.
- Support the Wellness Manager in the development, strategic planning, and effective implementation of programs that support quality improvement strategies, robust disease management programs, impactful population health initiatives, and seamless care coordination across the continuum of care.
- Identify key cost-drivers within the insured population and develop proactive plans and effective strategies to inform efficient care navigation processes.
- Assess and report on the tangible impact of wellness technical activities and programs within the broader context of wellness initiatives and active case management strategies.
- Conduct, in close collaboration with other dedicated team members, real-time analysis of emerging disease trends based on valuable claims data and provide timely advice on appropriate interventions to mitigate risks.
- Liaise and collaborate effectively with other relevant primary healthcare providers and key stakeholders across the public, non-government, and private sectors, as well as actively engage with consumers, in relation to the seamless coordination of care for clients and the successful implementation of population health interventions.
- Cultural Development: Foster a positive culture of empathy, professional excellence, and strong ethical conduct within the care navigation section, promoting a client-centric approach.
- Quality Improvement: Drive continuous improvement initiatives aimed at enhancing the overall effectiveness of care navigation services and achieving improved client outcomes through data-driven strategies.
- Uphold high ethical standards in all professional interactions with clients, healthcare providers, and internal colleagues, building trust and maintaining integrity.
- Stay informed about all relevant healthcare laws and regulations, including those specifically related to the insurance industry and the protection of client privacy.

- Maintain accurate and complete records of all client interactions and developed care plans, ensuring comprehensive documentation.
- Be transparent with clients regarding the services offered, any potential limitations, and all applicable fees or charges associated with care navigation services.
- Safeguard the strict confidentiality of all client information and consistently adhere to stringent privacy policies and procedures.
- Seek proper authorization in accordance with privacy regulations before disclosing any client-sensitive information to external parties.
- Proactively identify potential risks associated with care navigation activities and collaborate closely with relevant departments to implement effective mitigation strategies.
