Job Summary

The Social Worker will play a crucial role in providing psychosocial support to patients and their families. This role involves conducting assessments, providing counseling, advocating for patients' needs, and assisting with discharge planning. The Social Worker will work closely with the healthcare team to ensure that patients receive holistic care that addresses their emotional, social, and financial well-being.

Duties & Responsibilities

  • Psychosocial Assessments: Conduct thorough psychosocial assessments to identify patients' emotional, social, and financial needs.
  • Counseling and Support: Provide individual, family, and group counseling services to help patients and families cope with illness, hospitalization, and recovery.
  • Care Plan Development: Collaborate with the healthcare team to develop and implement patient-centered care plans.
  • Advocacy: Advocate for patients' rights and facilitate access to needed services, including financial assistance and community resources.
  • Discharge Planning: Assist in developing and executing discharge plans, coordinating with community agencies and home health services.
  • Communication Facilitation: Serve as a liaison between patients, families, and the healthcare team.

Qualifications & Requirements

  • Education & Experience:
